We need you to complete forms for your camper! These forms are available in your CampInTouch account under "Forms and Documents". All forms must be completed by May 3rd, 2024. Forms not completed by then will incur a $75 late fee per camper.
Health Forms
- Physician Exam Form - A form that your camper's pediatrician completes.
- Parent Authorization Form - Copies of your camper's health insurance card are added to this form, then complete, scan, upload. If you have more than 1 camper, you can create 1 form and in the top box where it says 'camper name, session, gender', you can add more than 1 child and then upload the 1 copy to each of your child's forms sections.
- Health History - Questions about your camper's overall health including vaccines, allergies, medications. Complete online.
- Food Allergy & Anaphylaxis Emergency Care Plan*
- COVID-19 Vaccination Card*- Upload an image of your camper's card including all eligible boosters. Scan and upload. Covid-19 vaccination is encouraged bur not required.
- Medication Letter For Your Physician*
- Hometown Pharmacy Registration*
- Scheduling Form 2024* - If you camper will be connecting over Zoom with their tutor or therapist. Complete online.

- Camper Application - Application to enroll your camper at Midwest. Complete online.
- Camper Photo - A picture of your camper, head shot style. Upload online.
- Bunk Request Form* - Complete if your camper is requesting to be with a friend. Complete online. -Optional
- Letter to my Counselor - Questions for your camper to complete to give their counselors an idea of who they are before meeting them. Complete online.
